When individuals refer to the noses of alcoholics, they are typically envisioning a red, swollen, or bumpy nose that appears to be related to too much drinking. Such is the perception that has been with us for decades. But the reality is more nuanced. The “alcoholic nose” that we commonly refer to is actually a skin disorder called rhinophyma

What is rhinophyma and why is it associated with alcohol?

The “noses of alcoholics” is not a medical term. It’s a name that became attached even though it’s not entirely accurate. Rhinophyma is a very severe version of rosacea, a chronic skin condition that induces redness, bumps, and in some cases, visible blood vessels on the face. With time, the skin on the nose will thicken and assume a bulb-like look. 

How do these changes in the nose occur?

Rhinophyma develops slowly. It typically begins with the general symptoms of rosacea — redness, sensitivity, and some swelling. As time elapses and if left untreated, the oil glands in the skin enlarge, and the skin becomes uneven and thick. This is more typical of elderly men, particularly those who are age 50 and beyond.

Can We Treat the Condition?

Yes. The sooner treatment is administered, the better the outcomes. In cases of mild to moderate symptoms, physicians can dispense:

  • Topical gels or creams that are anti-inflammatory.
  • Oral antibiotics to manage the bacterial activity and swelling.
  • Dermabrasion or laser therapy to eliminate excess tissue and to even out the skin.
  • Surgery in severe cases, to rebuild and recreate the nose and address aesthetics.

What is the actual role of alcohol?

Although alcohol is not the underlying cause, it is nonetheless a trigger. Alcohol consumption can be associated with increased facial redness and exacerbation of symptoms in some individuals. That’s where the association between alcohol and the condition originated.
